Admissions: Combination Degree

How to apply:

Combination Degree Instructions

Qualifying UF Majors: For on-campus UF undergraduates only. Qualifying majors: Music, Public Health, Chemistry, Biology, Microbiology, and Psychology.

  1. Combination Degree Request Form: Initiate contact with both undergraduate and graduate departments. 
  2. Letter of Recommendation: Submit one (1) letter of recommendation from faculty or professionals who can speak to your ability to be successful in a graduate program. 
  3.  CV/Resume: A section to upload your most recent resume. Please include any pertinent professional or volunteer experience with the arts, with health, or with arts in health.  
  4. Personal Statement: Tell us why you are interested in this degree, and what your career goals are. This two-page statement should describe (1) your goals and tell us how you envision this program will help you accomplish them, and (2) how you would facilitate or coordinate creative experiences for people in a healthcare and/or community setting to support health. The statement of purpose requested here may be the same as the one submitted with the online application.
  5. Writing Sample: All applicants must submit one piece of long-form writing, such as a research paper, essay, article, report, or proposal. The ideal writing sample has either been graded, published, submitted to an organization, peer-reviewed, or adheres to other professional or academic standards.
