Welcome to the UF Musician Referral Service Performance Registration Site. 

Please sign up for your performance at the UF&Shands Orthopedics and Sports Medicine Institute by providing the information requested below, and then click on the link to download the information page. This page will provide you with details about your performance including parking and contact information. You may select any weekday between the hours of 8am and 5pm for your performance.
